The school maintains a sick room staffed by a nurse each day from 8:00 - 3:10. Sick or injured students should be referred to the school nurse. The nurse plans programs to educate employees and students about health, safety and related issues, policies and procedures. The school nurse is the only employee who would ever administer medication to a student. Teachers and administrators receive CPR training and other training as may be appropriate.
All parents must complete a Medical Permission Form for each student prior to the start of the school year.
PDS follows the Tennessee Department of Health Rules for Immunization. All students must have an up-to-date Tennessee Certificate of Immunization to attend school.
The immunizations your son receives for Senior Kindergarten are the last ones he will have until he enters the 7th grade. If your son is re-enrolling for 1st-6th grade, you may ignore this section. The SK Certificate of Immunization is the last one you need to turn in while a student at PDS.
The state’s immunization schedule follows the current schedule published by the Centers for Disease Control and Prevention (CDC) and endorsed by the American Academy of Pediatrics (AAP) and American Academy of Family Physicians (AAFP).

Please either upload a scanned copy of your son's immunization record when you complete the online medical permission form, or mail or drop-off a copy at the school.
In order to better serve PDS students who must take medications during the school day, please assist us by following these guidelines which are recommended by the American Academy of Pediatrics Committee on School Health and the Memphis and Shelby County Health Department:
- Parents must bring the medication to the office. Children are not allowed to transport medication. The medication should be provided to the school in the original container labeled by the pharmacist. This label must include patient’s name, name of the drug, dosage, how often taken and reason for the medication. This label will serve as the doctor’s order. The medication will be kept in a locked space in the school nurse office. The school nurse will keep a record (date and time) of all medication administered.
- If a child is given a prescription to be taken “three times daily,” it is recommended that the parent give it to him before he comes to school, after going home from school, and before bedtime. The school will give medication only when dispensing is ordered more often or with lunch.
- It is the responsibility of the parents to get refills when the bottle is sent home.

Over the Counter Medications
PDS provides some over-the-counter medications, such as brand or generic versions of Advil, Tylenol, Benadryl, Tums and topical (skin) medications. The student will not be given any of these medications unless a permission form is filled out and on file in the nurses’ office.
A separate form is required for parent-provided oral, over-the-counter medications such as Mucinex, cough syrup and allergy medications like Zyrtec. If you would like your son to be given any of these medications you must bring the medication in the original bottle labeled with the student’s name and grade along with a completed permission form. This form may be picked up in the nurses’ office or completed online.
To ensure the safety and well-being of all PDS students, parents are requested to follow these important guidelines. If you have any questions or concerns regarding circumstances, please feel free to call the school nurse at 901–842–4627.
All medical concerns should be brought to our attention immediately. Please help us keep down the spread of illness by keeping your son at home when he is sick. If your son has vomited, had diarrhea, or had fever in the last twenty-four hours, he cannot be at school.
YOUR SON MUST BE FEVER FREE WITHOUT FEVER REDUCING MEDICINES AND WITHOUT MEDICATION TO CONTROL VOMITING AND/OR DIARRHEA FOR A FULL 24 HOURS BEFORE RETURNING TO SCHOOL.
He will be sent home if he has a fever or any other contagious illness, i.e. pinkeye, etc. We strongly encourage good hand washing - it is the best defense against children spreading germs to one another. Please send a note if he is to be excluded from physical activity (i.e. recess, gym); otherwise, we will expect him to participate in all activities.

FOOD ALLERGY POLICY
PDS recognizes that food allergies can cause serious life threatening conditions for some students. In an effort to keep our students safe so that they can focus on learning and development, PDS will strive to limit those foods that have been determined to cause allergic reactions in students, acknowledging that we cannot guarantee an allergen free environment.
Peanuts and tree nuts can be an especially difficult food to limit; however, PDS will strive to be nut sensitive.
PDS will strive to be nut free in the cafeteria programs and at all receptions, programs, or special events during normal school hours. Foods that are brought to school by families including those served at receptions, fundraisers, holiday parties, birthday celebrations or food that is to be shared with other students must be from the PDS pre-approved food list. Due to the risk of cross-contamination, foods prepared at home may not be brought to school for the purpose of serving other students. Individual students, faculty, and staff may bring nuts and nut products for lunch and snacks for their individual consumption only.
Each student with a diagnosed food allergy is strongly encouraged to have a comprehensive Individualized Health Plan developed to meet his health needs, including an Action Plan to treat possible reactions to food allergies.
The following procedures are based on The Food Allergy and Anaphylaxis Network’s School Guidelines for Managing Students with Food Allergies 2009.
FOOD ALLERGY PROTOCOLS AND PROCEDURES
THE FAMILY’S RESPONSIBILITY:
- Notify the school of the student’s allergies on the Health Form at the beginning of each year or if the allergy is diagnosed during the academic year, notify the school in writing as soon as possible.
- Work with the student’s physician and, to the extent necessary, the School Nurse, to develop an Individualized Health Plan that accommodates the student’s needs throughout the school including in the classroom, in the cafeteria, in after-care programs, and during school-sponsored activities.
- Work with the student’s physician and, to the extent necessary, the School Nurse, to develop a Food Allergy Action Plan.
- Provide written medical documentation, instructions and medications as directed by a physician, using the Food Allergy Action Plan as a guide. Include a current photo of the child for the Action Plan.
- Provide properly labeled medications and replace medications after use or upon expiration.
- Educate the student in the self-management of his food allergy including:
- safe and unsafe foods
- strategies for avoiding exposure to unsafe foods
- symptoms of allergic reactions
- how and when to tell an adult about an allergy-related problem
- how to read food labels (age appropriate)
- Review policies/procedures with the student’s physician and the student (age appropriate) after a reaction has occurred.
- Provide current emergency contact information, and update all contact information throughout the school year if there are changes in phone numbers or address.
- Separated or divorced parents must include applicable provisions of their Permanent Parenting Plan Order in the development of the Food Allergy Action Plan.
- The School Nurse and Division Head will review health records submitted by parents and physicians.
- The School will ask students, families, faculty and staff to provide only non-nut food options from the PDS pre-approved list when bringing special treats or food for receptions, birthdays, or special events. Due to the risk of cross-contamination, homemade goods should not be brought or served.
- The cafeteria manager will review all foods purchased for the cafeteria to ensure they are tree nut and peanut free and will check all off-site prepared foods served in the cafeteria to make sure it is tree nut and peanut free.
- There will be an Allergy Free Table designated in the cafeteria. Students with nut allergies may eat at this table. Others can eat at this table as long as their foods do not contain nuts or nut products.
- Faculty who regularly interact with the students will be trained by the School Nurse to understand food allergies, recognize symptoms, and know what to do in an emergency.
- The School Nurse will store the medications appropriately and will be sure that an emergency kit is available that contains a physician’s standing order for epinephrine. Medications will be kept in an easily accessible secure location central to designated school personnel, not in locked cupboards or drawers. A student will be allowed to carry his own epinephrine, if age appropriate, after approval from the student’s physician, parent and school nurse, and allowed by state or local regulations.
- Following an allergic reaction, the School Nurse will review the Food Allergy Action Plan with the parents and student (age appropriate), after a reaction has occurred.
- The School Nurse and/or the Faculty will discuss field trips with the family of the food-allergic student to decide appropriate strategies for managing the food allergy while the student is away from campus. Students and Faculty will be instructed not to bring snacks or treats that contain tree nuts or peanuts.
- Will not trade or share food with others.
- Will not eat anything with unknown ingredients or known to contain any allergen.
- Will be proactive in the care and management of his food allergies and reactions based on his developmental level.
- If the student is allowed to carry medications on his person for the treatment of control of an allergic reaction, he will not share, show or allow others to handle medications.
- Will notify an adult immediately if he eats something containing the food to which he is allergic.

Prior to issuing enrollment contracts for the 2016-2017 school year, PDS will review the total progress (not just academic) of each student. The school will then make the re-enrollment contracts available online for 2016-2017 in the early winter.
Re-enrollment contracts, along with a specified deposit, must be completed by the date specified on the re-enrollment contract in order to reserve a spot for the 2016-2017 school year.
Spots not reserved by the re-enrollment deadline will be released to new students when admission decisions are announced mid February.
The school may, in a handful of cases, elect not to issue a contract to a student, or it may choose to defer a decision on issuing a contract. The school may make such a decision for academic, developmental, behavioral, or other reasons. It may, as well, elect to issue a contract with certain stipulations.

PDS does not accept “cash” payments for tuition.
There is a Late Payment Fee of $50 if payment of the account balance is not received on or before the scheduled due date. Any balances more than 30 days past due will accrue interest at the rate of 1.0% per month.
There is a $50 Returned Payment fee for any check or direct payment (ACH) debit returned unpaid by any financial institution.
Delinquencies of more than 30 days may result in the suspension of the student treated as an unexcused absence. A boy with a delinquent account may not attend school in the next trimester without the written permission of the Headmaster, and any such absences will be treated as unexcused absences. A boy may not graduate with an unpaid balance or have any school record or recommendation sent to another school with an unpaid balance. Report cards may be held for boys with delinquent balances, and parent access to on-line grades may also be denied. Additionally, re-enrollment for any subsequent school year may not occur until all outstanding balances with respect to earlier school years shall have been paid in full.
If the school shall elect to take legal action to collect any unpaid balance, the person financially responsible for the student agrees to pay all expenses incurred (including, but not limited to, attorney’s fees, collection expenses and court costs, as well as pre-judgment and post-judgment interest).

Since the enrollment contract obligates parents for the full year’s tuition, we strongly recommend the Tuition Refund Plan in the unlikely event that a boy may leave PDS during the school year. Details of this insurance program and its cost will be emailed to all parents annually. *Please note that the Tuition Refund Plan does not become effective until the student has been in attendance for the first 14 days of the school year.
Enrollment for any trimester is contingent upon all tuition and fees having been paid when due. Charges for tuition and fees are for the full year. No reduction or remission of tuition and fees can be allowed for absence, withdrawal, or dismissal. No student may enter the school until first trimester tuition and fees are paid, and no student may go from one trimester to the next with an unpaid account.
Tuition for the 2019–2020 school year will be set by the Board of Trustees and announced by mid-December 2018 or at the latest early January of 2019.
PDS does not want to lose any student for financial reasons, so the school can and does provide significant tuition assistance resources to those families who qualify. To be a wise steward of its tuition assistance resources and to treat families equitably, the school uses an outside professional service (called Student Scholarship Services by NAIS, SSS) to determine which families need tuition assistance and how much these families need. The determination of SSS takes into account many factors, including income, number of children, medical and educational expenses, assets, debts, and so forth. Thus, families with wide-income ranges often qualify for tuition assistance.
The school will only make tuition assistance awards to those families who demonstrate a need. In granting these awards, PDS expects that families should make their son’s education a financial priority. While it is not our intention to manage a family’s financial choices or expect that families forgo modest vacations and so forth, tuition assistance awards from PDS will not subsidize extravagant lifestyles and/or overindulgent consumer consumption.
Families seeking a tuition assistance award for the 2016-2017 school year must have completed and submitted all of the required forms by November 10, 2015 This includes the following:
- A Parents Financial Statement (PFS) completed online through SSS (sss.nais.org)
- September and October pay stubs for both parents uploaded to SSS
- A copy of 2014 income tax return and W–2’s submitted to SSS if not aleady submitted
Tuition assistance awards will be announced in January before reenrollment contracts are due.
Awards will not be considered final until SSS has received the 2015 W–2 and 2015 income tax returns.
The school maintains a very confidential process for tuition assistance. Students who receive tuition assistance are expected to maintain satisfactory grades and behavior. Families receiving tuition assistance one year must apply for it the following year under the normal procedures for that year.
Families applying for tuition assistance after the deadline will likely find that all of the school’s tuition assistance resources have been committed and that they might only be placed on a waiting list should dollars become available.
When a boy enrolls at PDS, his family becomes part of the PDS community, and it is the strong, cohesive, and supportive nature of our community that helps make PDS the special school that it is. We are blessed to have a school in which parents and guardians are actively engaged in positive and constructive partnership with the school. As a school, we seek to build close bonds with our families; to be accessible, open, and user-friendly; and to assure that our relationships and dialogue with parents and guardians enable us to serve our boys as well as possible.
PDS needs and expects the positive and sustained cooperation and support of its parents and guardians if we are to be successful in serving our boys and achieving our mission. Parents and guardians must understand and embrace our mission and core values and support fully all teachers, administrators, and staff, as well as the entirety of the school’s curriculum, policies, and decisions. When united in shared purposes and beliefs, the parents and school form a powerful team with the profound ability to serve powerfully and positively individual boys and the whole school community. In short, there must be a positive and constructive relationship between parent(s)/guardian(s) and the school. Absent this partnership, a boy cannot be properly served, and damage may be done to him and/or the school community, and therefore such a boy and the school would be better served by his enrollment at another school for which there is a better fit.
We reserve the right to dismiss from the school a boy whose parent(s) or guardian(s) are unable, unwilling or incapable of understanding, embracing and supporting our mission, values, employees, curriculum, decisions or policies; whose actions undermine our ability or credibility to teach a boy(s); who model behavior and relationships that undermine the sort of behavior, relationships and values we are seeking to build in the boys of PDS; whose actions damage the school community; who do not address concerns, questions and problems in a constructive and respectful manner; or who demonstrate a lack of respect, trust, kindness and civility toward the school or for any employee of the school, or who violate any term of the enrollment contract or other rules, including in this handbook. Such a dismissal may be immediate and is at the sole discretion of the school administration.
Just as there are rules and expectations for boys, we hold them for parents and guardians also. In addition to the items listed earlier, parents and guardians should assure that their boy(s) arrives at school on time and that they have completed any work at home required by a teacher. Parents and guardians should attend conference days and special class events. They should familiarize themselves with the mission, philosophy, programs and policies of the school. They should follow all parental procedures.
